Please download and print the necessary camp documents below. The Medical and Waiver form must be filled out completely and provided at registration prior to any player participating in our training camps.
CONFIRMATION /REGISTRATION
Please fill out the necessary registration and payment information online and shortly after your application and payment have been completed; you will receive an acceptance and confirmation email that contains further detailed information.
EQUIPMENT NEEDED
Players must supply their own legal equipment in order to participate in camp. A camp jersey will be provided.

MEDICAL INSURANCE
EACH PARTICIPANT MUST HAVE THEIR OWN MEDICAL INSURANCE. Neither the coaches nor anyone associated with Dallas Developmental Hockey Inc., Coaching Camps and Clinics will be responsible for any medical or dental insurance. Any accidents or illness will be treated at a hospital near the rink. Our assumption of risk and agreement release must be signed before any participation may occur.
